Do you “do” Session Notes?

“Yes” COULD mean “Yes, I compose them for every session, and share them with clients as valuable touchstones from our conversation that are meant to be referenced again and again by them and me.”

It could also mean “Yes, I scribble down a few bits, never share them, and MIGHT review later.”

The former is transformational, whereas the latter barely means anything.  Same for action planning.  Same for tracking metrics.
